This case report was written up at the Department of Dermatology and Venereology, Government Medical College, Calicut, Kerala, India. The paper was published in the International Journal of Trichology.

Low thyroid hormones (hypothyroidism) has been known to cause thinning of the hair for a very long time.

“Hypothyroidism is a common and well recognised cause of diffuse hair loss.”

This type of thinning of the hair when combined with hair loss on the outer 3rd of the eyebrows is a textbook sign of hypothyroidism.

These days most doctors will provide synthetic thyroid hormone in the form of T4 (Synthroid) for the treatment of hypothyroidism.

But sometimes it doesn’t work – and there are a number of reasons for this.

One of the most overlooked reasons for this is that thyroid hormones need adequate zinc in order to work properly.

“Zinc and other trace elements are required for the synthesis of thyroid hormones.”

To complicate things even further, zinc is a very difficult mineral for the stomach to absorb.

We already don’t eat enough zinc and numerous things can decrease zinc absorption.

One of the things that prevents zinc absorption is hypothyroidism.

“Thyroid hormones are essential for the absorption of zinc, so hypothyroidism can result in zinc deficiency.”

The doctors at this medical college in India were quite aware of this combination of issues.

They treated a number of difficult cases.

They reported that both thyroid hormone supplementation and zinc supplementation may be required to reverse hair loss.

“The hair loss attributed to hypothyroidism may not improve with thyroid supplementation unless zinc supplements are added, as demonstrated in our case.”

They reported on a 28 year old woman who had lack of appetite, weakness, hair loss and scaly skin lesions.

She also demonstrated the textbook thinning of the eyebrows and eyelashes seen in hypothyroidism.

Thyroid hormone labs revealed that her thyroid hormones were low.

So she was given thyroid hormone supplementation.

Her thyroid hormone labs improved, but her hair and skin problems remained.

Zinc tests revealed she had inadequate zinc stores, so she was started on zinc supplementation

“Zinc monohydrate 140 mg capsules containing 50 mg of elemental zinc were given twice a day, along with other multivitamins.”

Only the combination of zinc and thyroid hormones lead to a reversal of her hair loss and related skin problems.

“One month later the skin lesions had cleared completely – a subsequent follow-up demonstrated complete regrowth of hair over a course of 4 months.

Zinc is a very important nutrient known only for the hair but also for the immune system.

Lower zinc levels are one of the reasons why older people have impaired immunity.

We don’t consume enough zinc – we don’t absorb enough – and many health problems which can affect digestion into even lower zinc levels.

The combination of hypothyroidism and unresolving hair loss can be an indicator that the person has insufficient zinc.

“The effect of hypothyroidism on the metabolism of zinc should not be overlooked. Zinc deficiency evaluation is warranted in all cases of hypothyroidism.”
