Is this a cure for ulcerative colitis?

Ulcerative colitis is a form of inflammatory bowel disease which can be difficult to treat.

A number of treatments can help to put it in remission and extend remission.

But these treatments can have severe side effects and eventual flare ups are very likely.

Over the years, a number of non-chemical treatments have proven to be rather effective.

One of these is a type of vitamin E called alpha-tocopherol, administered rectally.

1 human study showed that rectally administered high doses of vitamin E can put ulcerative colitis in remission in two-thirds of patients by month 3.

A follow-up study showed that these patients remained in remission if they continued treatment to the 1 year point.

In fact, 12 months on, 83% were in remission.

The human study was done at Amir-Alam Hospital in Tehran, Iran. The paper was published in the World Journal of Gastroenterology.

“We investigated the effects of rectal d-alpha (d-α) tocopherol administration on mild and moderately active ulcerative colitis.”

D-alpha-tocopherol is a form of vitamin E.

It has potent anti-inflammatory effects and can reduce many of the inflammatory mediators produced in inflammatory bowel diseases.

The researchers recruited a number of people with active ulcerative colitis for the study.

The therapy involved administering a gel/liquid form of high dose vitamin E into the rectum once a day.

The idea was to get large amounts of vitamin E where it was most needed – without significantly increasing blood levels of this nutrient.

“Fifteen patients with mild and moderately active ulcerative colitis were enrolled in an open-label study of d-α tocopherol enema (8000 U/d) for 12 wk.”

Assessments were carried out every month – this included endoscopes at the end of month one and at the end of month 3.

“Endoscopic evaluation was performed at baseline and after 4th and 12th weeks.”

The researchers assessed the effectiveness of the treatment using standard scoring techniques (DAI).

“Disease activity was measured with the Mayo disease activity index (DAI) and remission was defined as DAI of ≤ 2 with no blood in stool. Clinical response was defined as a DAI reduction of ≥ 2.”

This DAI score looks at a combination of stool frequency, rectal bleeding, mucosal health and the doctor’s rating of disease severity.

By week 4 there were good results.

“On the 4th week of therapy, 12 patients showed clinical response, 3 of whom (21.4%) achieved remission.”

But by week 12, the results were almost unbelievable.

“At the end of 12th week, the average DAI score significantly decreased compared to the beginning of the study.”

The DAI score dropped from 8 down to 2.3!

Two-thirds were in remission by month 3.

“After 12 wk, all 14 patients responded clinically to the therapy and remission was induced in 9 of them (64%).”

There were no significant adverse events reported due to the treatment.

The only downside was the inconvenience of performing the treatment itself.

“No patient reported adverse events or was hospitalized due to worsened disease activity.”

This initial study shows that this particular form of vitamin E at a dose of 8000 units per day could be very helpful for people with ulcerative colitis.

“This preliminary report suggests that rectal d-α tocopherol may represent a novel therapy for mild and moderately active UC.”

There are numerous reasons why vitamin E could help with inflammatory bowel diseases.

The anti-inflammatory effects are the most obvious.

“The observed results might be due to the anti-inflammatory and anti-oxidative properties of vitamin E.”

The same team reported a longer term follow-up of this study a little later.

“Herein we report the 12 months follow up results of the study.”

Over the follow-up period 3 of the initial 12 participants had left the study – but none left due to side effects or adverse events.

“Three patients have left the study since its beginning (1 on 3rd week, 2 after 12th week, due to personal reasons).”

There were significant improvements seen by the end of month 3 – and during the follow-up period none of the patients got worse.

In fact, remission rates increased from around 60% to over 80% at the one year point.

Disease activity remained stable during 12 months follow up, all 12 patients responded clinically to the therapy and remission was induced in 10 of them (83%).

By 3 months, most people were in remission (green zone). The benefits were retained with the therapy for the entire 12 month period.

Nobody in the study had a flareup over the course of 1 year.

This is extremely unlikely to happen at random – it is almost certainly due to the vitamin E treatment.

“Follow up results show no case of recurrence among patients after 12 months.”

All of the patients reported that the treatment itself was no big deal.

“There have not been complications and patients tolerate the procedure with no major discomfort.”

Taking this amount of vitamin E orally could lead to very high levels of vitamin E in the blood.

This might have a blood thinning effect which could be dangerous.

But the rectal route of administration used in this study did not lead to elevated levels of vitamin E in the blood.

“Plasma level of vitamin E was normal in patients.”

This method shows remarkable benefits in the management of ulcerative colitis.

“The follow up results suggest that vitamin E not only reduces the development of colon inflammation, but also prevents the recurrence of UC in a 12 months survey.”

You would always consult a healthcare practitioner by treating and diagnosing health-related problems.

It is particularly important to talk to your doctor about using high doses of vitamin E if you are taking other blood-thinning treatments.
