This is why I won’t ever get a cortisone shot

And if you are taking prednisone tablets or getting cortisone shots don’t stop without consulting with a doctor.

Prednisone tablets in particular often must have gradual reductions in the dosage in order to get off of them successfully.

And although steroid shots, like cortisone shots, can give nearly immediate relief from pain…

They can also have some bad, long-term side effects like faster progression of arthritis and the destruction of joints.

Because it is a significant risk factor that can dramatically speed up how fast a joint disease gets worse.

There is a lack of prospective and large retrospective studies evaluating potential joint findings, including increased risk for accelerated OA progression or adverse joint events, after treatment with IACS injection. Four main adverse joint findings have been structurally observed in patients after IACS injections: accelerated OA progression, subchondral insufficiency fracture, complications of osteonecrosis, and rapid joint destruction, including bone loss.

While the risk of RDHD following a single low-dose (≤40 mg) triamcinolone injection is low, the risk is higher following high-dose (≥80 mg) injection and multiple injections. These findings provide information that can be used to counsel patients about the risks associated with this common procedure. In addition, caution should be taken with intra-articular hip injections utilizing ≥80 mg of corticosteroid and multiple injections.

Researchers from Kaiser Permanente Hawaii tracked patients with rapidly destructive hip disease.

They found that if patients had previously had steroid injections in their hip joints they were 8x more likely to have rapid degeneration of that joint.

The damage happens in part because the steroids produce injury to the cartilage in the joints.

“With regard to biologic plausibility, the potential for corticosteroids to injure articular cartilage is well established. In a systematic review, Wernecke et al. concluded that intra- articular corticosteroid had a dose-dependent injurious effect on cartilage in vitro as well as in vivo. In the clinical setting, corticosteroid injections have been linked to an increased risk of osteoarthritic progression in the hip and knee.”

As with other types of treatments there are always trade offs.

But the tradeoffs for cortisone injections are bigger than what I’m personally comfortable with.

I want my joints to stay as healthy as possible throughout the rest of my life, so I’ll avoid these injections entirely.
