Hidden heart disease

A new set of studies reveals that many of us may have hidden heart disease and don’t know it. This is not detectable and yet it can prove fatal.

Let me explain.

In recent years there have been multiple headlines about a possible link between endurance exercise and heart attacks.

“a half-marathoner and two other runners died during the Detroit marathon on Sunday, organizers said”

 

“Sudden cardiac death” resulting from endurance events is a highly-debated topic.

Examining known markers of heart problems in people undertaking endurance exercise could help shed some light on the question.


In this study, scientists from several universities in the United Kingdom and Denmark investigated the effect of prolonged exercise on heart function.

The study recruited nine experienced male endurance athletes whose mean age was 33 years.

The experienced athletes were put through a half ironman triathlon.

A half ironman is a 1.9-km swim followed by a 90-km cycle and finishing with a 21.1-km run.

Blood was drawn from the participants before the study, after each of the three stages, and 24 hours after completion of the event.

Scientists looked at stress biomarkers known to be involved in heart attacks.

Cardiac troponin is a series of proteins involved in contraction of the heart muscle.

Increased cardiac troponin is used to diagnose heart problems in people reporting symptoms of heart disease.

It can help differentiate one type of heart problem from another.

“we tested the highly specific marker of cardiac muscle damage, cardiac troponin”

Prior to the study none of the participants had elevated cardiac troponin.

After phase 2, however, a couple of the participants had elevated levels.

After phase 3, the number of participants with elevated cardiac troponin levels rose to 4.

“cardiac troponin was elevated in four subjects over the course of the half ironman triathlon”

24 hours after the event, one subject still had elevated cardiac troponin.

That’s not a good sign.

Creatine kinase was another blood marker they looked at.

Elevated creatine kinase is a marker for heart attack risk.

Creatine kinase levels increased after every stage of the event.

“significant increases in creatine kinase were observed during and after the half ironman triathlon”

24 hours after the event, creatine kinase decreased slightly.

However, creatine kinase was still elevated by more than 70% – a day later.

“twenty-four hours post-exercise, total creatine kinase was still statistically elevated”

Scientists then looked at the electrical impulses from the heart.

After each exercise stage, alterations in these impulses were looked at on an electrocardiogram (ECG).

The scientists observed changes in the heart’s electrical signals, indicating changes in how the heart was pumping blood.

These changes may indicate elevated risk.

Overall, this study found many changes in heart parameters over the course of this multi-hour endurance event.

Though some changes returned to normal after 24 hours, others did not.

A review of the scientific literature from 1979 to 2010 questions the recent hype about endurance events and sudden cardiac death.

The authors found that there may be a correlation between existing heart conditions and sudden death during endurance events.

“Most deaths are due to underlying heart disease. The value of pre-screening is limited by its insensitivity and the impracticality of widespread implementation.”

This review backed up the study discussed above.

It noted that blood samples and electrocardiogram data from other studies show abnormalities during and after endurance events.

Statistics in the review indicate that risk increases after the 20-mile mark.

Where heart health is concerned, less may be more…

Exercise is beneficial to our health… when we can recover from it.

This data might indicate that exercise should be treated the same way medical treatments are:
